Discharge Procedures

   CGH’s discharge time is at 11:30am (before lunch). Additional charges may apply for discharges after 11:30am and a full day charge may apply for discharges after 5:00pm. 


Accessing your discharge summary

  • To streamline your discharge process, your discharge summary will be available in Health Buddy within a working day of discharge.
  • The discharge summary includes informations such as admission details, diagnosis, and presciptions, for your ease of reference.

  • You can also request access for your loved one's discharge summary on Health Buddy.
  • As part of our sustainability efforts, hardcopies discharge documents will no longer be provided. If preferred, you may request for hardcopies here. Please note that each hardcopy request will incur a charge.

Follow-up Appointments

  • Upon discharge, an outpatient follow-up appointment may be arranged. You may be given a follow-up appointment at CGH or referred to a polyclinic.
  • For hospital appointments, find out how to manage your appointments here.
  • For polyclinic appointments, please book an appointment with your preferred polyclinic in advance. 
  • Do bring your doctor’s memo, identification documents and any prescribed medical products (eg. wound care products) for your follow-up appointment.

Collection of Medicine

  • Upon discharge, your prescription will be electronically sent to our pharmacies. Do collect your medication before going home. Please refer to the table below for the pharmacy you should collect your medications from.
  • If your next-of-kin is collecting your medication on your behalf, please ensure that they have your NRIC, as well as ward and bed details.
  • As part of our sustainability efforts, you are encouraged to bring your own bag for medication collection.

If you were discharged from The Integrated Building, please proceed to:

Functional Assessment Report (FAR)

The Functional Assessment Report (FAR) evaluates the need for assistance in Activities of Daily Living. The assessment is required when applying for government support schemes administered by Agency for Integrated Care (AIC), including the Pioneer Generation Disability Assistance Scheme, Home Caregiving Grant, and the Migrant Domestic Worker Levy Concession for Persons with Disabilities.

Charges apply for the FAR at all healthcare institutions, regardless of the outcome of scheme application. Do note that this cannot be claimed under Medisave or MediShield Life, and will be included in your final bill.
