The Neighbours for Active Living (Neighbours) programme was set up in 2013 to ensure that patients with complex health and social care needs receive continued care in the community after their discharge from hospital.

The Eastern Community Health Centre (Eastern CHC) (Tampines) supports and complements health services provided by General Practitioners (GPs) in the management of medical care in the community.

Tampines Family Medicine Clinic

Tampines Family Medicine Clinic (Tampines FMC) is a partnership between Changi General Hospital and a private GP partner.

CareLine

CareLine is a 24/7 personal care telephone service by Changi General Hospital (CGH). It provides health and social support to seniors who may be living alone or are frail, and helps these seniors to keep safe at home.

GPFirst

The GPFirst programme encourages people with mild to moderate medical conditions to visit their General Practitioners (GPs) first, rather than going directly to the emergency department.

Community Hearing Clinics

The Changi General Hospital Community Hearing Clinics provide second level hearing assessment and support for seniors.
