Community Nursing

SingHealth's Community Nurses are part of SingHealth's Healthier SG (HSG) Teams, providing place-based integrated care and empowering residents to achieve their health goals.

Community Nursing services in the Eastern region of Singapore are provided by nurses from Changi General Hospital, serving these six precincts: Bedok North (includes Bedok Reservoir, Kaki Bukit, Kembangan), Bedok South (includes Bayshore, Frankel, Siglap), Geylang East (includes Kampong Ubi), Pasir Ris-Changi, Tampines East and Tampines North/West. Together with Neighbours for Active Living (Wellbeing Coordinators), they provide the following services for residents:

Community Health Posts

CGH Community Nurses are stationed at Community Health Posts that are sited conveniently at the Active Ageing Hubs and Centres, Community Clubs, Residents’ Networks and faith-based organisations listed below. They serve residents who:

• Are aged 40 years old and above
