If you are a patient at CGH and are keen to engage in an ACP discussion, approach a member of your care team (e.g. doctor in-charge or nurse) during your clinic visit, inpatient stay or home visit.
The ACP discussion will guide you to:
- Reflect on your personal beliefs, values and share your healthcare preferences with your loved ones and healthcare providers.
You will be asked to:

Appoint a nominated healthcare spokesperson (NHS)
- To help you to make important decisions in the event you are no longer able to communicate.
- Discussions will be documented
- This is to guide your healthcare team and loved ones to make decisions on your behalf and in your best interests during a medical crisis.
