A medical report is a comprehensive summary of a patient’s medical condition. Medical report requests are not included in the consultation fees as they require doctors to set aside additional time beyond clinic sessions to conduct detailed reviews. As such, medical reports are separately charged.

Requests for medical reports may be cancelled and fees refunded only if the report has not entered the processing stage. Once the doctor has begun reviewing the patient’s medical records or if any document (e.g. certified true copies, test results, duplicates of medical certificates/discharge summaries) has been prepared, the request is considered in progress, and fees are applicable.
To cancel your request, please submit the completed Withdrawal of Medical Report Application form by mail or email. If you prefer a refund via bank transfer, please include a bank statement showing the bank logo, recipients account name and number.

| Court Appointment for Deputy Report | This report is prepared by the assigned psychiatrist in response to requests that require detailed professional opinions not covered by a simple report. The report covers an assessment of the patient’s condition, which may extend to domains of prognosis and disability. Ordinary Medical Reports are usually processed within 4 to 6 weeks. Processing may take longer if:
Specialist Medical Reports require a longer processing time as the patient may need to be reviewed at the Specialist Outpatient Clinic after discharge or has been given an open date for clinic review.
Completed reports will be sent to the applicant’s email address or mailing address (as provided in the Application and Consent for Release of Medical Information Form) by SingPost Tracked letterbox.
