Ward AdmissionTo indicate your preferred payment arrangement and submit the related supporting documents for your clinic visit.

For submission of the completed MediSave Claim Authorisation Form (Single) and supporting documents for your visit.
For submission of the MediSave Claim Authorisation Form (Multiple) authorisation electronically via HealthHub. Please email billing@cgh.com.sg after you have completed the authorisation if you would like to use MediSave for your visit.
