The Geriatric Medicine Centre, located in Changi General Hospital (CGH), is a one-stop facility that provides specialised care to cater to the complex medical, nursing and rehabilitative needs of older adults.

The center is made up of the Geriatric Specialist Outpatient Clinics (SOC), as well as the only Geriatric Day Hospital in Singapore.

Geriatric Specialist Outpatient Clinics

The Geriatric Outpatient clinics provide outpatient care to elderly patients with multiple medical problems and geriatric conditions such as falls, incontinence and memory impairment. There are also specialty clinics for the management of elderly with Parkinson’s and stroke disease.

Besides geriatric care, Psychogeriatric Medicine clinics provide management of significant psychiatric or behavioural disturbances caused by age related brain impairment or mental illness in older patients

Geriatric Day Hospital

The Geriatric Day Hospital (GDH) offers an outpatient, short term programme for frail elderly who require a comprehensive and multidimensional approach. The programme focuses on medical stabilisation, functional restoration, caregiver education and support for frail elders so as to promote independence, longer maintenance at home and reduce caregiver burden.

The GDH is staffed by a team of skilled nurses and therapists, supervised by a geriatrician and supported by dietetic and social services. Patients attend full day sessions on a weekly basis for a total of 12 weeks and they may eventually be discharged to a community - based service as deemed appropriate.

To enroll in the Geriatric Day Hospital programme, patients should first be assessed by a geriatrician for suitability. If you are not currently seeing a geriatrician at CGH, you may wish to ask your physician to refer you for an assessment.
