Request for a new appointment by filling up an online form. Please allow us 2 working days to process your appointment and contact you back for confirmation.

For urgent and emergency cases, please call 995 or go to an emergency room.

You can now receive your appointment details for the next 60 days via SMS.
To retrieve your appointment details, send the following:

CGHAPPT[Space]NRIC/HRN/FIN Number[Space]DOB in DD/MM/YYYY to 77227
