A subgroup of Telecarers provide educational calls to patients above 60 years old after their discharge from the A&E department.


How HMU supports patients

A patient with a certain medical and surgical condition is first identified, either through an admission to the hospital, or through a follow-up appointment. The patient will then be automatically assigned to a Telecarer by the system, andthe Telecarer will guide the patient to manage his or her medical condition. Telecarers cover topics relevant to the patient’s condition, medications, the diet or lifestyle adjustments, how to monitor the condition and what red flags to look out for.

HMU also works closely with the patients’ primary care doctor or specialist. This ensures that patients remain well in the community.
