Rehabilitation Nursing

The goal of Rehabilitation Nursing is to assist patients in attaining and maintaining their maximum function. This model of care empowers patients to live as normally as possible in a hospital, complete with comprehensive rehabilitation facilities, to optimise recovery and facilitate independent living before returning home.

Rehabilitation Nurses work closely with a multi-disciplinary team which includes rehabilitation physicians and allied health professionals. They provide input in team-decision making on patient treatment goals and discharge plans in weekly multi-disciplinary team meetings. The team performs functional assessment for patients on admission and weekly, using functional independent measurement tools and implements a care plan by providing direct nursing care and education to maintain and restore patient function.

The nurses provide a safe and therapeutic environment, and support various activities like Wii games, play, art and music therapies, and nurse-led exercise programs that promote patients’ functional recovery.
