Health Management Unit

The Health Management Unit (HMU) was conceived with the view to engage patients with certain medical and surgical conditions in the community, and to equip them with the knowledge and skill through tele-education. Patients are usually more receptive to listen and learn about their conditions in the comfort of their own homes.

HMU’s tele-education is supported by a customer relationship management system called Patient Relationship Management (PRM). The HMU Telecarers (Registered Nurses) are able to access past clinical indicators, as well as capture each interaction with the patient. The system allows Telecarers to have an overview of the patient’s condition before calling, thus structuring a personalised conversation which provides appropriate support to better meet patient’s needs.
