Request for Medical Report

Who can Request for a Medical Report?

  • Patients aged 21 years and above.
  • A patient who is a minor may apply for a medical report with the consent of his/her parent or legal guardian.
  • If the patient is deceased or has lost mental capacity, consent is required from the appointed representative of the estate. If there is no representative appointed for the estate, a separate Letter of Undertaking has to be completed by the next-of-kin of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration, as proof of your relationship to the patient must be submitted together with the application.
  • Interested third parties, with the consent of the patient.

How do I submit my request?

What are the Charges?

This is a factual report of the patient's medical condition. A doctor will prepare the medical report based on patient's available medical information. (Insurance form or type-written report).

Similar to the above. This will be completed by CGH psychiatrists only.

​Specialist Medical Report/Permanent Disability Form

​This report is prepared by the patient's specialist in response to requests that require a professional opinion with regard to the patient's prognosis and disabilities. It is based on an actual assessment of the patient’s condition and may involve a review at the Specialist Outpatient Clinic (Insurance form or type-written report).

​Court Appointment for Deputy Report

​This report is prepared by the assigned psychiatrist in response to requests that require detailed professional opinions not covered by a simple report. The report covers an assessment of the patient’s condition, which may extend to domains of prognosis and disability. The requirement for further assessment for the purpose of preparing the report is subject to review by the assigned/ treating psychiatrist at the Specialist Outpatient Clinic.

​Work Injury Compensation Form (Initial / Re-Assessment)

This is an assessment to determine work-related injuries, including the degree and period of disability for workmen’s compensation, under the Work Injury Compensation Act. The scope of the report is as listed in the "Medical Report on Traumatic Injuries for Workmen's Compensation" form prescribed by the Ministry of Manpower. This may involve a review at the Specialist Outpatient Clinic.

Note:
The “Medical Report on Traumatic Injuries for Workmen’s Compensation” form needs to be obtained from the Ministry of Manpower.

​Work Injury Compensation Medical Board Assessment

This is a referral from the Ministry of Manpower to assess and re-determine work-related injuries, including the degree and period of disability when any of the parties (insurer, employer or injured worker) object to the results of the initial work injury compensation assessment. The scope of the report is as listed in the "Referral of Objection to Permanent Incapacity Under Compensation (Medical Board) Regulations 2005 - Medical Report on Traumatic Injuries for Workmen's Compensation" form prescribed by the Ministry of Manpower.

​Admin Fee for Duplication of Medical Certificate (per copy)

A certified true copy of the Medical Certificate issued by the Emergency Department, clinics and wards.

​Admin Fee for Duplication of Discharge Summary / Investigation Results / Referral letter​

A duplicate copy of the Hospital Discharge Summary with brief information on the surgery, diagnosis and procedure or selective duplicate copies of investigation results such as blood tests can be made available.

*Admin fee charges may differ depending on the nature of the request.

​Note: Medical report fees are chargeable according to discipline. Additional charges such as consultation fees, radiological and laboratory investigation charges that may be incurred in the preparation of the medical report will be borne by the applicant.

Timeline for Request Completion

Ordinary Medical Reports are usually processed within 4 to 6 weeks.

Delays in prompt processing may be due to the following reasons:

  • Patient has upcoming clinic appointment(s) or has been warded
  • Reports requested from several clinical departments
  • The doctor is away on leave

Specialist Medical Reports require a longer processing time as a review at the Specialist Outpatient Clinic may be required after the patient has been discharged or given an open date for clinic review.

Completed reports will be sent to the email address or mailing address (as provided in the Application and Consent for Release of Medical Information Form) by SingPost Tracked letterbox.
