Neighbours for Active Living

 

The Neighbours for Active Living (Neighbours) programme was set up in 2013 to ensure that patients with complex health and social care needs receive continued care in the community after their discharge from hospital.

The Neighbours team consists of professionals with social work and nursing backgrounds. They support the care needs of CGH patients in the community to prevent readmissions to hospital, and work in close partnership with our Changi General Hospital (CGH) Community Nurses as part of the CGH Integrated Community Care Team.

What we do

  • Holistic assessment of patient’s health and social care needs
  • Identify and coordinate services and activities for patients with complex medical, social and care needs
  • Conduct regular home visits to assess patient’s condition and safety
  • Provide long term monitoring and psychosocial support
  • Support patients through health coaching to set and achieve their health goals

Our patients

 
  • Patients who are discharged from hospital and require healthcare and/or social services coordination, but lack psychosocial support.
  • Patients who require transport, meals, medical escort and/or healthcare assistance.
  • Patients who require long-term assistance by Intermediate and long term care (ILTC) providers, but there is a service gap in the community, which does not fully meet their needs.
  • Caregivers who require respite support to help manage their loved one’s care at home, and can benefit from regular visits by CCTs.
  • Caregivers and patients who need support to cope with the care at home and move patients towards self-management.
  • Isolated patients who would benefit from befriender support.

Our Milestones

Neighbours has touched the lives of more than 13,000 patients to date. The 10th year commemorative publication celebrates the journey of Neighbours, sharing heart-warming stories from staff, community partners, volunteers and patients.
