Community Psychogeriatric Programme (CPGP)

The Community Psychogeriatric Programme (CPGP) is dedicated to improving the mental health of older persons living in the community. As a community outreach programme, we aim to accomplish this through partnerships with other caregivers in the community in order to promote ageing in place for older persons. We value the importance of empowering eldercare agencies and caregivers with the capability to holistically enhance the well-being of older persons.

Our Aims

To increase awareness of mental health in older persons through education and the provision of mental health screening as required.

To improve the quality of life of older persons through early detection and effective and timely treatment of mental health problems.

To foster partnerships with eldercare agencies to meet the psychosocial needs of older persons in the community.

To empower community eldercare agencies with the knowledge and skills to enhance the provision of care to older persons with mental illness.

Our Team

As part of Geriatric Psychiatry Service within the Department of Psychological Medicine, the CPGP team is multidisciplinary, consisting of psychiatrists, doctors, nurses, psychologists, medical social worker, occupational therapists, physiotherapist and admin support staff.

Our Partners

  • Eldercare agencies such as Senior Care Centres, Home Care, Day Care Centres and Nursing Homes.
  • Primary care physicians and other specialists.

Our Services

Training and Support

We provide training, consultation and support for eldercare agencies and primary care physicians through:

  • Multidisciplinary training programmes
  • Case discussions
  • Networking

Clinical Visits

We deliver home based assessments and interventions for older persons with mental health conditions.

Our interventions include:

  • Pharmacological, psychological and social intervention(s).
  • Support for caregivers.

Referral Information for Clinical Visits

Clinical visits are only for those residing in the selected areas and meet the following criteria:

  • Aged 65 years and above with mental disorder(s)
  • Who are unable to access hospital or outpatient services

Exclusion criteria for clinical visits:

  • Older persons with physical disabilities only

Some common psychological disorders in the elderly include:

  • Anxiety disorders
  • Adjustment disorders
  • Dementia with behavioural issues
  • Grief and loss issues
  • Mood disorders such as depression
  • Psychotic disorders

The referral has to be initiated by a healthcare professional.

Please fax this completed form to 6787 3013. The referring source will be informed of the outcome of the referral. Appropriate charges apply for clinical visits.

Closed on Saturday, Sunday and Public holidays
