Our Clients
Neighbours’ clients are typically sick and frail elderly who lack a good support system to help care for them at home. For example, some may live alone or their caregivers are themselves suffering from multiple health and social conditions. As result, many develop complications, suffer unnecessary relapses and are repeatedly admitted to hospital.
They are referred to the Neighbours team by grassroots activists who are familiar with residents in the area, step-down care providers such as the Senior Activity Centres or even other residents who are concerned that their neighbours may need assistance. Once the Community Care team is alerted, they will step in to assess a potential client’s situation and follow-up by referring them for further medical attention and/or assigning volunteers to monitor them.
