CGH@Home


CGH@Home offers hospital-quality care to patients in the comfort of their homes. As part of the national Mobile Inpatient Care-at-Home (MIC@Home) programme, clinically suitable patients can receive comprehensive medical attention without the need to be admitted to hospital.


Our multi-disciplinary care team provides treatments for a range of specialties, including:
  • General Medicine
  • Geriatric Medicine
  • Cardiology
  • Emergency Medicine
  • Orthopaedic Surgery
  • Post-Operative Surgical Care

Eligibility

Admission to CGH@Home is based on professional medical assessment. The attending physician will initiate the referral process after evaluating the patient's medical condition. 

Considerations include:
  • Clinically stable and care needs can be provided for through CGH@Home
  • Able to perform activities of daily living (ADL) independently, or have a caregiver to assist with ADLs
  • Safe and hygienic home environment
  • Access to a phone for teleconsultations

Team-based care

  • Multi-disciplinary care by doctors and nurses experienced in home-based care, supported by specialists and allied health professionals including physiotherapists and medical social workers 
  • 24/7 clinical care and support from care team through teleconsultations
  • Combination of daily teleconsultations and home visits to administer treatment
  • Real-time monitoring and response to alerts on abnormal vital signs or deteriorating conditions 
  • Established care escalation protocols
  • Doorstep delivery for medications via Medication Delivery Service
