Geriatric Day Hospital
Elderly
patients who need special medical follow-up attention after
discharge may enrol in CGH's Geriatric Day Hospital . This
is an innovative facility that provides affordable medical,
rehabilitative and nursing care to elderly patients. Patients
at the Geriatric Centre attend individualised daily rehabilitative
care programmes and return home in the evening. Our staff
also trains the patient's caregiver, e.g. a family member
or maid, in home care skills.
The Geriatric Day Hospital complements home
care for the elderly patients. It is also useful for elderly
patients who require multi-disciplinary assessment. Patients
above the age of 65 with either one or more of the following
conditions are particularly suitable:
- Multiple medical problems
- Bladder and bowel problems, e.g.
incontinence or constipation
- Mobility difficulty, e.g. stroke,
Parkinson's disease, arthritis, old fractures etc
The Geriatric Day Hospital is situated on
Level 2, next to the Specialist Dental Clinic.
To enrol with the Geriatric Day Hospital, patients should
first be assessed at the Geriatric Medicine specialist clinics.
Please ask your geriatrician at CGH if you are suitable for the programme. If you are not seeing a geriatrician at CGH, you may wish to ask your current physician to refer you to a geriatrician for assessment first.
Please note that this is not a long-term placement and we aim for most of our patients to be eventually discharged to day rehabilitation centres within the community.
