Programmes

Following a series of focus group discussions with our patients and their carers, Changi General Hospital recognised that patients who need care at home, faced a lot of difficulties. Carers often did not know what was required of them.

To help some of our patients and their families cope with post-hospitalisation care, CGH developed the Skills for Life: HomeCare programme. The aim is to provide patients and/or the family carers with the knowledge and skills to manage care at home.

Group Training

This training is competency-based and is conducted by our specialists including physiotherapists, occupational therapists, nurses, pharmacists and medical social workers. As our training involves hands-on practices, there is a maximum limit of 15 participants per session.

Personalised Training (For Inpatients Only)

This is a customised hands-on training programme tailored just for you. It is personalised and conducted by our physiotherapists, occupational therapists and qualified nurses.

Home Assessment / Visit

To assess your home environment, visits are conducted by one of our physiotherapists or occupational therapists. They will advise you on necessary modifications to your home, and the equipment or items that may assist you in home care.

Option One

Home visit by our occupational therapist / physiotherapist

Cost: $80 per visit

Option Two

Video-tape your home environment and show us the video
Advice on how to modify your house
Cost: Free

Training Materials / Book

A training manual has been produced to complement the skills taught in the programme.

VCD

To reach out to a wider audience, a Video Compact Disc (VCD) version of the HomeCare Training book has been developed. The VCD serves as a teaching aid and helps accelerate the learning process.

The HomeCare Place

The HomeCare Place, located on level 1, CGH, helps the elderly, chronically ill and physically challenged patients to live a quality life at home after being discharged from the hospital. You can find a variety of affordable self-help equipment such as special utensils, crutches, wheelchair and commodes. These items are designed to assist the users so that they may lead a more meaningful and independent life. Our staff specially trained in homecare will be able to provide advice on what is suitable for you.
