APPOINTMENT BOOKING

Making a clinic appointment can be a breeze. Just log on to the e-appointment page on the website and our staff will contact you by the next working day. You can also get a list of consultation charges at our specialist clinics.

Baffled by any symptoms, signs or other health-related concerns? This service is dedicated to address your health-related concerns or any health issues that may interest you. Our team of experienced nurses, in consultation with a team of medical specialists and paramedical professionals, will provide you with the information that you need.

MEDICAL REPORTS

Here, the Medical Records Office provides a one-stop service for medical report processing and duplication of x-rays. The reports you can request for include insurance claims, workmen compensation, specialist and 2nd opinion. If you are above 21 years of age, simply submit online the consent form for the release of medical information with the appropriate fee. For those below 21 years of age, parental consent is required.

UPDATE OF ADDRESS

You can inform us of your change in address simply by filling in the form and furnishing us with your new and old addresses. Just fill in your name, NRIC, old address, new address and your contact number. Send the information to us and we will contact you for the confirmation.
