How HomeCare Assist Helps

HomeCare Assist helps needy ill or bedridden patients when they are discharged from the hospital. The needs required are real. The hospital's team of social workers, therapists, nurses and doctors evaluate the needs of each patient and will provide a basket of help according to their circumstances.

We estimate that each patient who is assisted in the programme will need $1000 a year.

The patients' needs include :

  • Home modifications to make the home more friendly for those who are bedridden or wheelchair bound. This include installing grab bars and commodes, converting squatting to sitting toilets and widening of doorways.
  • Medical Equipment such as oxygen concentrators, hospital beds and nebulisers to enable the patients to be cared for at home
  • Consummable Supplies such as diapers, feeding tubes, milk feeds and dressing that are regularly expended and pose a financial strain on the family
  • Transport Fares incurred for regular attendance at clinics or day care centres

To complement other services already provided by VWOs, we will subsequently refer these patients for long-term support and assistance. Patients and their families will be means tested to determine their eligibility.
