How You Can Help?

HomeCare Assist aims to help needy ill or bedridden patients who need help to stay at home with their family after discharge from the hospital. Often due to the lack of homecare items such as diapers, milk feeds and nebulisers, these patients are poorly cared for at home.

To be eligible to apply for assistance, a person must be currently staying at home and meet the conditions in a means test.

Application forms are available from Medical Social Services, or alternatively, you can print out the application form from your computer and fax it us at 6784 4984 or by post to:

Cheque Donation

Your donation can help the needy patients of HomeCare Assist. If you wish to help by making a cheque donation, please make out the cheque to "SingHealth Foundation - CGH", and mail it to :
