Request for Medical Report 
Our Medical Records Office (MRO) provides a one-stop service for the request of medical reports and completion of insurance claims forms. The release of medical information is subject to approval by the hospital.

Patient’s written consent is required for the release of medical information. For patients below 21 years of age, the consent of patient’s parent is required. If the patient is deceased / incompetent, consent is required from the appointed representative of the estate. If there is no representative appointed for the estate, a separate Letter of Undertaking has to be completed by all family members of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration, as proof of your relationship to the deceased must be submitted together with the application.

How do I Apply for a Medical Report?

  • Submit online Request for Medical Report
  • Write to us and enclose a crossed cheque payable to “Changi General Hospital Pte Ltd” with the appropriate medical report fee

How soon can I obtain the requested medical information?

Ordinary Medical reports are usually processed in 3 to 4 weeks from time of request.

Delays in prompt processing may be due to the following reasons:
     - Patient has upcoming clinic appointments
     - Patient has been hospitalised 
     - Patient has multiple reports requested from several clinical departments
     - The doctor is away on leave

Specialist Medical Reports and Workmen Compensation cases require more processing time as a review at the Specialist Outpatient Clinic may be required after the patient has been discharged or given open date from clinic review.

Requests for duplication of investigation results will be completed within 1 week of receipt at MRO.


How will I Receive the Completed Medical Report?

Patients can request for the medical report to be

  • Mailed to them
    • Mailing address to be provided at the point of application.
    • An additional fee of $2 is payable if report is sent by local registered mail.

  • Collected personally at MRO
    • Contact number to be provided at the point of application.
    • Patient will be contacted as soon as the report is ready for collection.
    • NRIC of patient to be produced upon collection.
    • Authorisation letter stating the collector’s name and NRIC number is required if a third party is collecting on the patient’s behalf.

For online applications, you will need to come personally, as our staff needs to verify patient’s consent during the collection of the medical report. Please bring along your NRIC.

Provides more detailed information on the diagnosis, procedure conducted, the medical conditions and/or treatment given.

Ordinary Medical Report
A factual report on the patient's medical condition written by a doctor based on patient’s available information.

Specialist Medical Report
A report written by the specialist that provides a specific opinion or prognosis. It is based on an actual assessment of the patient and may require a review at the Specialist Outpatient Clinic.

MOM Workmen's Compensation Form
This is for determining work-related injuries and the degree and period of disability. The doctor will fill up the MOM form "Medical Report on Traumatic injuries for Workmen's Compensation" and this may involve a review of the patient at the Specialist Outpatient Clinic. The review will be scheduled after the patient has been discharged from his medical appointment at the clinic.

MOM Workmen's Compensation Objection Form
This is for re-determining work-related injuries and the degree and period of disability when patient objects to the results of the first workmen's compensation report raised. This involves a review at the Specialist Outpatient Clinic and the report is completed by two Consultants.

Patients are required to provide our Consultants with their previous medical report or investigation results if their first assessment is not conducted at our hospital.

Second Opinion Medical Report
This is for non-CGH patients who seek our Specialist's professional opinion. It is written based on the assessment of the patient and a review at the Specialist Outpatient Clinic. Patients are required to provide the attending specialist their previous medical report or investigation results.

Note: Medical report fees are chargeable as per discipline and 7% GST is included in the medical report fees stated above. Additional charges will be borne by the patient if further tests such as x-ray or laboratory procedures are required for the completion of the medical reports.
